About WACH Program

About Us

At WACH, we’re dedicated to combating the high prevalence of heart disease, including conditions like high blood pressure, high cholesterol, and stroke, which disproportionately affect West Alabama. With heart disease being the leading cause of death in the US and stroke ranking as the 5th leading cause, our mission is clear: to pioneer new and innovative approaches aimed at reducing risk factors and improving heart health outcomes.

Primary Goals

Our primary goals at WACH are structured around forging partnerships in each community, deploying dedicated Community Health Workers (CHWs) to lead learning sessions and facilitate group activities, and encouraging the use of Alabama One Health Record. Through these initiatives, we aim to create a network of support that addresses barriers within our targeted populations, provide tailored care through CHWs, and enhance communication and management of patient heart health using Alabama’s health information exchange.

How We Work

At WACH, we work closely with our community to understand the factors influencing health. This includes diet, stress, weight, exercise, medication and pregnancy. We help people make healthier choices and improve heart health.
